Blood Donation system

the system should have doner and that doner must have personal information specially ssn(social security number) and status that status dedicate if he is has disease or not and hospitals where the operation of blood donation done every hospital have (hospital number and name and location ) and in the system there doctors have (name and phone) work in hospital and must be there a supervisor on the doctors , before the doner do the donation he must give to the doctors a blood sample to be check in if there is any disease in the blood sample.
