Clinical Pharmacy Forms

As part of my role supervising multiple hospitals, I developed and implemented two essential clinical pharmacy documentation tools:

1. A Mini Pharmaceutical Care Plan (Mini PCP) tailored for short-stay ICU patients to ensure efficient therapeutic documentation.

2. A Clinical Pharmacist Recommendation Form designed to be attached to the patient’s file, allowing for formal communication of medication-related interventions and recommendations.

Both tools enhanced documentation efficiency, improved multidisciplinary communication, and were adopted across all hospitals under my supervision.
