this task is not only to create form in clean code, but to fill in the blanks in the design. The challenge here is to provide a great user experience while keeping good coding standards.

Ground rules:

Assume only modern browsers will be used

The use of javascript libraries (or lack of thereof)

The form should be validated client-side

Use animations should be judicious, good taste will also be scored

Should work well both on mobile and desktop
